PROFESSIONAL REQUIREMENTS:

  • NYS RN
  • Long Term Care, Adult Subacute or Outpatient Experience Preferred
  • Pro-Active and Self Directed
  • Strong Clinical + Patient Centered
  • EMR

RESPONSIBILITIES:

  • Assessments, treatment plans, implement therapy strategies, re-evaluate progress to goals
  • Work closely with parents, teachers, clinical team;  attends meetings
  • Provide patient and family education on physical therapy, injury prevention, and self-care
  • Maintain accurate and up-to-date patient records and documentation

    DESCRIPTION:

    • Overall management + leadership of Skilled Nursing Facility of approximately 130-beds.
    • Mentorship and ability to hold staff accountable

    COMPENSATION PACKAGE:
