  • Min. 5 Years of healthcare-related experience.
  • Preferred Prior Care Management or Case Management experience in a hospital setting.
  • Min. PRI and/or Screen certification of intent to pursue within 6 months.  Must be able to travel to multiple locations on any given day as requested.   • LCSW or LMSW Required
