Required Skills & Work History

  • LCSW or LMSW required
  • Current LTC, SNF, Subacute experience is a MUST!
  • SigmaneCare EMR is a MUST!
  • Strong Customer Service Attitude
  • Excellent Admissions Assessment Skills
  • Discharge planning is a mandatory requirement for this busy Subacute!
  • Preferred (not required) - Bilingual Spanish or Russian language skills greatly appreciated.

  • Ability to keep beds filled.
  • PRI Savvy.
