QUALIFICATIONS:
- BSN
- NYS RN
- CCM
JOB REQUIREMENTS:
- Min. 5 Years of healthcare-related experience.
- Preferred Prior Care Management or Case Management experience in a hospital setting.
- Min. PRI and/or Screen certification of intent to pursue within 6 months. Must be able to travel to multiple locations on any given day as requested. Demonstrated ability to handle confidential information with discretion and ability to deal with the public in a professional and courteous manner. Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling.
- Min. Microsoft Office systems, i.e. Excel, PowerPoint, and internal communications.
