QUALIFICATIONS:
- LCSW or LMSW Required
- Social work experience: 3 years min. (Required)
- Long Term Care: 2 years min (Required)
- Social Work Leadership: 1-year min. (Required)
- Collaborative clinical team experience
- Comfortable with EMR software
